Investigators are continuing to piece together the sequence of events at UPMC Memorial Hospital that led to the deaths of a West York Borough police officer and the 49-year-old who held ICU staff hostage.

Lester Mendoza, who specializes in pulmonary critical care, was present when the perpetrator was informed about his loved one's death.

The media relations team with the hospital did not answer direct questions about safety and security, but some employees have spoken out about what they have noticed.
